Minor Participant Consent & Parent/Guardian Agreement

This Minor Participant Consent and Parent/Guardian Agreement ("Agreement") governs the participation of individuals under 18 years of age ("Minor") in D³ productions. This Agreement must be executed by the Minor's parent or legal guardian ("Guardian"). By executing this Agreement, Guardian consents to Minor's participation and agrees to be bound by its terms.

1. Guardian Consent

Guardian consents to Minor's participation in the D³ Production identified in the applicable application. Guardian represents and warrants that Guardian is Minor's parent or legal guardian with authority to consent to Minor's participation.

2. Guardian Obligations

Guardian agrees to be present or available during Minor's participation as required by D³, ensure Minor complies with all Production requirements and conduct standards, disclose any medical conditions, allergies, or special needs that may affect Minor's participation, maintain adequate health insurance for Minor during the Production, and comply with all applicable child labor laws.

3. License to Use Minor's Likeness

Guardian grants D³ a perpetual, irrevocable, worldwide, royalty-free license to use Minor's name, image, voice, appearance, and biographical information in connection with the Production, including for promotional and archival purposes.

4. Child Labor Compliance

D³ complies with applicable federal and state child labor laws, including laws governing working hours, rest periods, and on-set education requirements for minor performers. Guardian agrees to cooperate with D³'s compliance efforts.

5. Release of Claims

Guardian releases D³ from any and all claims arising from Minor's participation in the Production, including claims arising from injury, property damage, or other harm, except claims arising from D³'s gross negligence or intentional misconduct. This release is on behalf of Guardian and Minor.

6. Confidentiality

Guardian agrees to keep confidential all non-public information about the Production and agrees to ensure Minor does the same.

7. Governing Law

This Agreement is governed by the laws of the State of Texas.
